Before Your Visit

After you schedule your appointment, you will receive an intake packet by email with consent forms, medical history, contact information, and other required documents.

All paperwork must be completed at least 48 hours before your appointment. Incomplete paperwork may result in your appointment being rescheduled.

For all telehealth appointments, please have the following available:

• Home blood pressure monitor
• Scale for current weight

Payments

Insured Patients & Self-Pay

Intracoastal Behavioral Health works with many major insurance plans and also welcomes self-pay patients. Please contact our office to confirm coverage in Arizona or Florida.

Payment is due at the time of your appointment.

Out-of-Network / Superbills

If we are not in-network with your insurance, we are happy to provide a superbill at no additional cost. You may submit it to your insurance company or HSA/FSA for possible reimbursement. Please contact your insurance carrier to determine your out-of-network benefits.

A credit or debit card is required to reserve your appointment. Your card will only be charged after your evaluation is completed, for late cancellations, or for missed appointments.

Cancellation Policy

• Appointments canceled 24 hours or more before the scheduled time will receive a full refund of the deposit.
• Appointments canceled with less than 24 hours’ notice, or missed without notice, will incur a $75 cancellation / no-show fee.

Medication Refills

Medication adjustments are common during the early stages of treatment to ensure the safest and most effective outcome.

Patients will receive enough medication to last until their next scheduled follow-up. To avoid interruptions in treatment, please keep all scheduled appointments.

Prescription Policy

Intracoastal Behavioral Health is committed to prescribing medications safely and responsibly.

Controlled substances are prescribed only when medically appropriate and in accordance with DEA regulations and applicable state law.

We do not prescribe:

• Xanax (alprazolam)
• Ambien (zolpidem)
• Opioid pain medications

We do prescribe many evidence-based psychiatric medications, including:

• Antidepressants
• Anti-anxiety medications
• Mood stabilizers
• Other clinically appropriate psychiatric medications

If a controlled medication is not appropriate, we will discuss safe and effective alternatives.

Controlled Substance Policy

If controlled medications are prescribed, the following policies apply:

• Prescriptions are issued only during scheduled appointments.
• Early refills are not provided.
• Lost prescriptions will not be replaced.
• Stolen prescriptions require a police report before replacement will be considered.
• Long-acting stimulants are preferred when medically appropriate. Short-acting stimulants are generally not prescribed.
• A urine drug screen is required before the first controlled substance prescription.
• Annual drug screening is required, with additional testing if clinically indicated.
• All Schedule II, III, and IV medications are reviewed through your state’s Prescription Drug Monitoring Program (PDMP).
• The prescribing provider has final discretion regarding all controlled substance prescriptions.

Forms & Administrative Fees

For established patients:

• Medical records: $1.00 per page
• FMLA / disability forms: $100

Additional requests—including letters, reports, phone consultations, emails, or other administrative services—may incur fees based on the time required to complete them.
